Job Description 

The Case Manager will report directly to the Psychosocial Program Manager. S/he will collaborate with the Psychosocial Program Manager across TYO’s programs to identify families’ most critical needs and design customized intervention plans. Depending on specific needs, the Case Manager will also connect families with educational, financial, medical, security, and mental health resources in the Nablus Governorate and throughout Palestine. The Case Manager will advance TYO’s holistic, systemic efforts to promote individual, family, and community well-being. Like all TYO staff, the Case Manager will dedicate energy towards improving the social-emotional well-being of the Nablus community.

What You'll Be Doing (Essential Duties)

  1. Connect families and individuals with community resources based on unique needs.
  2. Collaborate regularly with the Psychosocial Program Manager to identify and adjust families’ needs and services. 
  3. Maintain relationships with educational, financial, medical, security, and mental health institutions in the Nablus Governorate and throughout Palestine. 
  4. Research and stay current about the processes for accessing and maintaining critical educational, financial, medical, security, and mental health services in the Nablus Governorate and throughout Palestine. 
  5. Develop treatment plans tailored to families’ unique educational, financial, medical, security, and mental health needs. 
  6. Coordinate with the Psychosocial Program Manager to monitor families’ treatment plans.
  7. Communicate with the Psychosocial Program Manager to share research and adapt and revise families’ treatment plans on a regular basis. 
  8. Gather and analyze data about families’ progress. 
  9. Communicate with the Psychosocial Program Manager to monitor, evaluate, and report about job tasks and outreach activities.
  10. Coordinate with the Psychosocial Program Manager to report about program observations and families’ ongoing needs.
  11. Plan and implement psychology/social work practicum for youth ages 20-35.
  12. Collaborate with the Psychosocial Program Manager to develop training materials for psychology and social work practicum. 
  13. Lead training sessions in case management techniques.
  14. Mentor participants about further opportunities to build social work expertise.

Qualifications

  1. BA in social work, community development, psychology, education, or related field; MA preferred.
  2. Knowledge about different educational, financial, medical, security, and mental health resources available in the Nablus Governorate and throughout Palestine.  
  3. Knowledge about social work, psychology, and other mental health training programs available in Palestine. 
  4. 3-5 years providing educational, financial, medical, security, and/or mental health services to disadvantaged populations, preferably in the West Bank of Palestine. 
  5. Experience developing and implementing social work trainings for youth ages 20-35.
  6. Experience collaborating with social workers, psychologists, and other mental health professionals to discuss specific cases and accomplish common goals. 
  7. Experience triaging cases to prioritize families and individuals with the greatest needs.
  8. Experience monitoring, evaluating, and reporting about multiple cases on a regular basis.
  9. Strong interpersonal communication skills.
  10. Ability to multi-task and provide high-quality services for multiple cases at once. 
  11. Strong organizational and short- and long-term planning skills. 
  12. Ability to use evaluation tools and data to analyze families’ strengths and weaknesses. 
  13. Clear commitment to TYO’s mission to empower disadvantaged children, youth, and parents to realize their potential as healthy, active and responsible family and community members.
  14. Fluency in Arabic and excellent written and spoken English.
